There are two ways to sign up to the lottery...
1) Download a membership form and send your form, together with your payment, to:Nottinghamshire Hospice Lottery, 384 Woodborough Road, Nottingham, NG3 4JF.
Payment can be by cheque made out to Nottinghamshire Hospice Ltd or by credit/debit card. Simply fill out your credit/debit card details on the membership form.
