Hospice at Home

Hospice at Home is a service for people in the latter stages of their illness who need nursing care and support in the home environment. This also provides periods of respite and support for the carer.

Referrals to the service are made by the GP, District Nurse or Community Nurse Specialists, or via the Palliative Care Referrals Centre, if the patient is City based.

For referral to the Hospice at Home service, a patient's prognosis should be 6 months or less.


What to Expect

A team of Registered and very experienced Auxiliary Nurses offers practical care and emotional support for varying periods over the 24 hour day, 365 days a year.

The nurse’s visit is usually between four and nine hours according to availability and complements the care given by the District Nursing Service.

This enables carers to take a much needed break, reassured that someone else is there to take over, for one day or night a week or more as the need arises.

The role of the nurses is to provide holistic care to the patient. Some of the nursing staff have qualifications in Complementary Therapies which is available upon request for both the patient and the carer.

Professional Services Provided

The Hospice at Home service is organised by the Co-ordinating team which is based at the Hospice

All Hospice at Homes Nurses are experienced in Palliative Care. We have both Registered and Auxiliary Nurses available.

The Hospice at Home team works closely with District Nurses and GPs to ensure high quality, professional care is delivered to meet individual needs.

The Nursing team also meet regularly with their Manager for support meetings and attend study days to ensure they are kept fully up to date with medical advances. Training and development is supported and encouraged by the Hospice.
