Hospice at Home

Hospice at Home is a service for people in the latter stages of their illness who need nursing care and support in the home environment. This also provides periods of respite and support for the carer.

REFERRALS - All referrals to the Hospice at Home service come through the District Nursing Team, Community Matrons or Community based specialist nurses for chronic disease.

For referral to the Hospice at Home service, a patient's prognosis should be 6 months or less.

What to expect
What to expect
A team of Registered and very experienced Auxiliary Nurses offers practical care and emotional support for varying periods over the 24 hour day, 365 days a year.

The Hospice at Home service is organised by a team of Hospice at Home Co-ordinators under the supervision of the Hospice at Home Manager.
