This is Joanne’s story and how our Hospice in Your Home team supported her mum, Maureen, to stay at home until the very end.

“It was important to us that Mum died at home. We couldn’t think of anything worse than all of us not being around her, talking to her, in her own surroundings, peaceful, calm. It was everything that we needed.” – Joanne

Where would you want to spend your last moments?

For most of us, home is where we feel safe, it’s where we feel most comfortable, it’s where we have created cherished memories. It’s where we want to be when we die.

But right now, too many families in Nottinghamshire are facing the end of life without the care they need. Only half of people who die across Nottinghamshire are able to die in their own home.

We enable 96% of our patients to die at home.

Demand for our care is growing rapidly. We urgently need to provide more care out in the community now. With rising costs and funding cuts, it’s getting harder to meet that need.

A regular gift today will help us plan for the future, so we can support more families when they need us.

‘We Enable 96% of patients to die at home’

Being there for the moments that really matter

When Mark’s condition deteriorated, he and his wife Jane were supported by our Hospice in Your Home team, so that they could make the most of every special moment on their daughter Lizzie’s wedding day.

Then, when Mark reached the point where he needed full time care, that same overnight care meant Mark could remain at home, in the centre of family life and the normal comings and goings of home.

““Knowing you played a part in respecting a person’s wish to die at home, and being their advocate when they are no longer able to communicate for themselves, is so important and a privilege to be able to do.” – Debbie, Registered Nurse.”

Debbie joined the Hospice last year. As a Registered Nurse in Charge, she coordinates care for patients ensuring they get the right support that meets their needs.
